There are no FDA-approved testosterone products for women in the United States, even though global guidelines, including the Menopause Society, recognize testosteroneâs effectiveness for treating low libido in post-menopausal women.
So your doctor has a few off-label options.
The most common is a male testosterone gel used at 1/10th of the male dose. Itâs a very small amount, relatively affordable, and one of the options I use in practice.
There are also other off-label options including compounded testosterone cream and androgel pump used twice weekly.
A lot of women are never told these options exist.

Itâs called Genitourinary Syndrome of Menopause (GSM). As estrogen levels drop, labial tissue can lose volume, flatten, or even seem to be âabsorbed.â
The good news? Itâs treatable, especially if you catch it early.

So many women experience bleeding after menopause and are not quite sure what to do with it.
ACOG just dropped updated guidelines on how this should be evaluated.
Hereâs the nuance:
If you just started menopause hormone therapy or recently had a dose adjustment and are within the first 3 to 6 months, some bleeding can happen while your body adjusts.
But if you are postmenopausal and not on hormones, or you are well past that adjustment window and still experiencing bleeding, this is the kind of bleeding that warrants full investigation.
And the standard of care just changed.
ACOG now recommends both a transvaginal ultrasound and an endometrial biopsy. Previously, one or the other would often suffice. That approach was missing up to 12% of endometrial cancers.

DHEA is a natural hormone that your body converts to both estrogen and testosterone.
A 2024 study found that vaginal DHEA can improve vaginal dryness, pain with sex, and distress from genitourinary symptoms.
Itâs an FDA-approved option for women who prefer to use something other than vaginal estrogen. And importantly, vaginal DHEA does not increase the risk of breast cancer.
If vaginal estrogen is something you prefer not to use, vaginal DHEA may be an alternative.
Have you heard of vaginal DHEA? Ask your doctor if this is an option for you.

Yes, I hate to be the one to break it to you, but your vagina does atrophy during menopause.
One of the biggest things I see patients struggle with during perimenopause and menopause is vaginal atrophy.
When estrogen drops, the labia thin and lose elasticity. The whole area becomes more sensitive and fragile, and some women start to notice changes over time.
Starting vaginal estrogen at the first signs of discomfort can help preserve that tissue. Itâs low dose and, for most women, does not carry the same risk as oral or systemic estradiol.
Itâs safe, effective, and one of the most underutilized treatments in womenâs health.
If youâre in perimenopause or menopause and starting to experience vaginal symptoms, talk to your doctor about whether this could be an option for you.

@acog_org , where ob-gyns shape the future of womenâs health, celebrated its 75th anniversary 𼳠in DC this weekend and Alloy was there, helping shape the conversation around menopause.
Last year our Chief Medical Advisor @smalonemd delivered the keynote speech. This year, her longtime friend @michelleobama took the stage. Our prescribing physicians were on the floor and at the podium, leading education for ob-gyns across the country. @dr.kudzai spoke on health disparities and the solutions women deserve access to now.
The energy at our booth said everything. This is why we show up. The standard for this stage of life is officially being raised, and weâre proud to be part of that.
Being featured in USA TODAY and sharing a story this personal reminded me just how often even physicians can miss whatâs happening in their own bodies.
Perimenopause doesnât always look the way we expect it to.
And for Black women especially, itâs often missed, dismissed, or misunderstood.
We deserve better.
Better awareness.
Better conversations.
Better care.
