So… you’ve torn your ACL. Now comes the big question: Do you need surgery, or can you go down the conservative rehab route? The answer isn’t as simple as “ACL tear = reconstruction.” Some people can restore stability, strength and function through a structured rehab-first approach. Others may be better suited to ACL reconstruction, especially when there’s repeated giving way, associated injury, or a goal of returning to high-demand pivoting sport. The key is looking beyond the scan. It comes down to: * How the knee behaves * What other structures are involved * What you want to return to * How well the knee responds to early rehab * Personal factors Surgery or no surgery, one thing stays the same: Rehab is non-negotiable. Different pathway. Same goal — build a knee that can handle the demands ahead. 5 WEEKS AFTER FOOT FRACTURE ↓ This was Sav at week 5 after a fractured cuboid in her right foot. These fractures often get written off as ankle sprains, and if missed, might make for a complicated return to sport. Five weeks in, here was how she was looking: • Walking completely pain free in and out of the boot • Holding her balance on one leg without any issues • Trialled double leg calf raises without any pain • Full ankle range of movement on both sides Here’s what the session looked like: • Manual therapy to reduce tone in the calf and shin • Gradual pain-free exposure to single leg balance work out of the boot to start trusting the foot again • Calf raise progressions at smaller amplitudes to get the muscles around her foot and calf back up to speed • Foot and ankle control drills to rebuild dynamic stability • Weaning off the boot in daily activities She got a repeat X-ray at the six week mark, which showed bone healing giving us more confidence to progress. We are monitoring her response to gradual loading very closely, making sure that she performs everything at a pain-free level with clean technique. Her goal with @eastcoasteaglesafl isn’t to get her back in a rush, but ready and confident. Cuboid fractures are manageable. They just need time, the right plan, and someone paying attention.​​

Hamstring strains are one of the most common and most mismanaged injuries in sport. Loading, timing, return to activity criteria. Juggling these can be challenging. Understanding how the hamstring gets injured, respecting pain as a guide rather than a stop sign, and following a structured progression back to full speed is what actually keeps athletes on the field long term. In two weeks I'm clearing my brother to play basketball again. I should feel good about that. And I do. But I'd be lying if I said there wasn't a part of me that keeps finding reasons to wait. His testing is strong. His movement is clean. By every measure I use to make this call, he's ready. If he walked into my clinic as a stranger with these exact numbers, I would have cleared him weeks ago. But he's not a stranger. He's Koby. And that changes everything. The hardest part of this job isn't knowing the science. It's separating what you know from what you feel. It's recognising when clinical caution stops and fear - your own fear - starts taking over. I'm his physio. But I'm also his brother. And the thought of being the person who sends him back too early and watches him go down again **touch wood** keeps me up at night in a way no other patient ever has. He's ready. I know he's ready. I just have to let him go prove it. 💬 Has anyone else had to make a hard call for someone they love? I'd love to know I'm not alone in this.
