The Fitzpatrick scale (still the global standard) was designed in 1975 for white European patients to assess sunburn risk. And South Asian skin is one of the least studied groups in dermatology and aesthetic medicine. Glutathione is one of the most important antioxidants in the human body. But IV glutathione delivery sold as the gold standard for skin brightening? That I am heavily not in support of, because the pharmacokinetics and safety don’t support it. Here’s the short version: 🔬 IV glutathione is degraded so fast in circulation that plasma levels normalise within minutes. It does not arrive at the skin intact. 🔬 The brightening effect attributed to IV glut is likely driven by cysteine, which your body can generate from N-acetyl cysteine (NAC), oral glutathione precursors, and topical actives that modulate melanogenesis at the enzymatic level. 🔬 The WHO has raised concerns. The FDA has issued warnings. The Philippine FDA banned IV glutathione for skin whitening. These aren’t fringe positions. If you’re spending hundreds per session on IV glutathione and seeing results, I want you to genuinely ask: what else changed? Your diet, your sun habits, your skincare? Your SPF is working exactly as designed. The problem is it wasn’t designed with melanin rich skin in mind. SPF measures UVB. Broad spectrum adds some UVA. What neither meaningfully addresses: long wavelength UVA1 and visible light, the wavelengths responsible for persistent pigmentation and melasma recurrence in darker skin tones. She came to me with severe hypopigmentation after an IPL session went wrong. Multiple dermatologists couldn’t help her. Not because they don’t care but because there are documented gaps in dermatology education around skin of color, and melanin-rich skin often pays the price. Here’s the thing: hyperpigmentation gets all the airtime in skincare conversations. But our skin is equally vulnerable to the opposite; hypopigmentation, loss of colour. And often, it’s not a genetic condition. It’s iatrogenic: inappropriate energy-based device use. IPL. Laser hair removal. Treatments that weren’t calibrated for melanin density. Many people think they’re purchasing a pure mineral sunscreen, in fact many think they’ve hit the jackpot with formulas that don’t leave a white cast. Especially people looking for sunscreen for hyperpigmentation or melasma, with darker skin. I previously said I do not believe no white cast is achievable for skin of colour with pure mineral sunscreens, and stand by that. Many formulas unbeknownst to consumers are not pure mineral. Even when they list the sole active ingredient as a mineral filter. Often, digging a little deeper into the ingredient list you will find spf boosting actives, chemical filters. There is nothing wrong with this. In fact, lowering zinc oxide concentration is one meaningful way to reduce white cast. And I personally do not think there is anything wrong with chemical filters or boosters. But this is not a true mineral sunscreen, it’s a hybrid formula. Designed to be more cosmetically elegant. And reduce white cast. SPF boosters are needed because usually zinc oxide in lower concentrations cannot reach meaningful very high protection sunscreen claims. At the end of the day the sunscreen you wear is the best one.
