I started hormone therapy when I was 17. Like many trans women in Malaysia, I didn’t begin through a doctor or structured gender-affirming care. I started using birth control pills, because that was the only information available to me at the time. It wasn’t ideal for hormone therapy. But when access doesn’t exist, people still find ways to survive. This is why harm reduction matters. In HIV response, we understand that people deserve safer options ,not judgment even when systems fail them. The same principle applies to gender-affirming care. Hormones affect the liver, heart, metabolism, and mental wellbeing. Without monitoring, risks increase ,not because trans people are irresponsible, but because healthcare access remains limited. When trans communities can safely access healthcare: ✅ we monitor hormones safely ✅ we build trust with providers ✅ we are more likely to test for HIV ✅ we stay connected to prevention and treatment services My journey taught me this: Access to affirming healthcare is HIV harm reduction. Seeking checkups isn’t fear. It’s self-protection. It’s community protection. We don’t reduce harm by denying care. I literally cried while typing this caption. All I can say is, I am not alone. Advocating for my transnita community can feel isolating, draining, and lonely at times. But I continue because of my love and responsibility to my community. Today reminded me of something important. When one voice speaks, others will rise. The stigma and discrimination our community faces is real, in media, in systems, and in everyday life. But today, we showed what collective voice looks like. These women didn’t just post. They amplified voices that are often unheard. Trans women who are excluded, unsupported, and still fighting for access and dignity. This is bigger than us. This is about community. And I couldn’t be more proud. Thank you for standing up. Thank you for showing up.
