Last week, a patient presented with chronic urinary incontinence, even after doing very consistent pelvic floor internal physiotherapy and HRT. The imaging was normal, so we knew a major structural problem was not at the root of the issue. After functional tests showed limited hip rotation, we palpated a very tight obturator internus (OI) muscle. The pelvic floor’s levator ani muscles have attachments to OI’s fascia at the tendinous arch, seen in this picture. In NKT, we know how antagonists can also be very tight. We found adductors to be very tight, which move the hip into internal rotation to oppose OI. We tested the relationship between these two antagonists. We found we needed to sedate the tension in the adductors with an active stretch, which allowed the OI to externally rotate the hip while reducing pelvic floor tension and without letting compensations occur. This common dysfunctional relationship can be found in incomplete hip treatments with pelvic floor symptoms. The patient found great relief and improved urge incontinence in the days following the session, so we had the homework mimic our effective NKT session for a minimum 2 weeks along with continuing pelvic floor therapy. The caffeine part is definitely under control. (No more than 200 mg daily.) The genius? I never had that. But the care - dissection has helped me care for a donor. This has helped me care for myself and spread that care to others. That’s why I am so proud and so very lucky to be an anatomist. 17 hours in the lab yesterday - then another 6 this morning - and it will never quite be enough discovery. Can’t wait to see what I learn next week from our donor. #anatomy #dissection

As a fellow equine enthusiast, this was a fun one for me. A dressage rider came in frustrated after struggling with her horse consistently falling in during the left lead canter. She also had persistent left adductor pain that just wouldn’t settle down. In riding, we talk constantly about “inside leg to outside rein.” Creating bend and balance requires both sides of the system working together. But what happens when the rider’s body can’t create that balance? Using NKT and ID assessments (ID 1 and 6), we found: * An intrinsic core deficit creating poor stabilization * A dysfunctional lateral subsystem pattern * Left adductor longus overworking for an inhibited/underworking right QL For equestrians, think of it this way: If your inside leg is doing all the work, but your outside rein isn’t supporting and receiving the energy, you’ll struggle to create true bend and balance in the horse. Her body was doing the same thing. The left adductor kept trying to create stability and connection, while the right QL (the “outside rein” of the system) wasn’t doing its job. The result was compensation, asymmetry, pain, and difficulty controlling the canter. Once we restored better core and lateral subsystem function, things changed quickly. A few days later she called and said her riding improved dramatically. She had better control in the left lead canter, a more balanced seat, and a much clearer connection with her horse. You can’t create balance in the horse if you can’t first create balance in the rider. Sometimes in life, you get really lucky. But I thought for 37 years I was unlucky in love. Each time my relationships would end, I would try to learn from it and move on swiftly. Somewhere in the midst of heartbreak, I believed deep down that I would find it. I did - on Dec 19, 2015. Just a few months later, he married me on May 7, 2016. That was ten years ago today. Jonathan traveled cross-country to move in with me just two weeks after we met. And we would have married each other right then upon meeting, that I know for sure. We managed to wait a few months, and people tried to warn us about the speed. I understood why they did. But there was a clarity upon meeting him. He felt that same clarity. Jonathan didn’t ever propose. I didn’t, either. We basically just said, “Yep - when should we do it?” And I’ve been lucky ever since. How do you give back to the universe, when you’ve been so lucky? I try to put that luck into everything thing I do. Every student, every patient, every friendship, everything. I want to show the universe I am grateful, so I give them my focus. I want them to feel lucky that we are in this effort together. For a decade, I have been loved by my favorite person - and I’m overwhelmed to tears. From the depths of my heart and soul, I hope you can love all the way to the people lucky enough to have found you. Happy 10th anniversary to my absolutely wonderful husband, Jonathan. I’m one lucky lady!
