Please take a moment to read the following from Hannah, one of our community members: 'Over the past few years, my mum has been visiting our great aunt — Ciocia — every week at a local Polish care home, where she received end-of-life care after developing dementia. I would visit too whenever I returned home from university. Watching someone you love slowly change due to dementia is incredibly difficult, and it’s something that stays with you. Ciocia Mila was a brave, courageous, and incredibly kind woman. She touched so many lives and was known as “Ciocia” (Aunty) to everyone — so much so that many people thought it was her real name! Her life was one of strength and resilience, growing up during the Second World War in Poland, the fled to Austria with her mother and then displaced in Italy for a while. Eventually after living in South Africa for a while she settled in London, where she worked in the International Institute of Strategic Studies, as well as doing a lot of voluntary work for Polish charity organisations. Despite life’s challenges, she remained bright and full of spirit. Even as dementia took hold following a fall during COVID, her feisty and headstrong personality continued to shine through. Ciocia Mila sadly passed away this December, leaving behind many cherished memories for all who knew her. We are incredibly grateful for the time we had with her. Through our personal experiences — both through work and recently other family members — we have seen how deeply dementia affects not only individuals, but entire families. It can be overwhelming, isolating, and heartbreaking. That’s why we want to do something to help. This June, my mum and I are going to be completing a Sprint Triathlon to raise awareness for dementia and support those affected by it. We are raising funds to support dementia awareness, improve access to care, and contribute towards vital research that can change lives. Every donation, no matter how small, will help support those living with dementia and the people who care for them. Thank you so much for your support — it truly means the world to us!' After 10 years of providing care across Hammersmith and Chiswick, we’ve seen the same fact repeat itself over and over: most hospital admissions are preventable. Here is exactly how we’ve kept our clients safe, happy, and at home for over a decade: 1. The Hydration Secret It sounds too simple, but it’s the #1 reason for falls. Dehydration leads to low blood pressure, which leads to dizziness. If you feel "wobbly" in the morning, drink a full glass of water before you even stand up. 2. Master Your Meds The "I’ll take it later" trap is dangerous. Missing a dose or doubling up is one of the fastest ways to end up in A&E. Use a monitored dosage box or, better yet, have someone check in. Consistency is the only way to stay stable. 3. "Audit" Your Floors Your home should be your sanctuary, not an obstacle course. That rug you’ve had for 20 years? If the corner is turned up, it’s a trip hazard. Those loose slippers? They’re a risk. Clear the walkways. Lighting is your best friend—keep the halls bright. 4. The Power of "Small and Often" If you stop moving, the world gets smaller. You don't need a gym; you just need to keep the "engine" running. A short walk to the shops on Chiswick High Road or just moving around the garden keeps your muscles awake. Use it or lose it. 5. Don’t Socially Isolate Health isn't just physical. When we stop seeing people, we stop eating well and we stop noticing the small symptoms. Connection keeps your mind sharp and your spirit high.

For Jess, care means having someone who understands what matters most to her. 💚 Her Care Professional supports her daily life with kindness and patience, whether it’s helping at home, joining her on outings or simply sharing a chat over tea. It’s care that gives comfort, confidence and connection every day. If someone you care about could use a little extra support at home, we’re here to help.

You might recognise the friendly face who visits — but there’s a whole local team behind every moment of quality care. From planning visits to keeping families informed, our office team works alongside our Care Professionals to make sure every visit runs smoothly, and every client feels supported. It’s just one of the things that makes care with Home Instead feel different.
