New patients can learn how to establish care and find providers accepting new patients on our website. Existing patients can schedule routine visits, request refills, or message their care team through the patient portal.

During this visit, you’ll meet with your care team to review your medical history, assess health risks, complete preventive screenings, and create a personalized plan to help you stay healthy.
This visit is not a physical exam and is covered by Medicare with no out-of-pocket cost. Eligible once per year after enrolling in Medicare Part B.
